Established in 1980, the Greater Lawrence Family Health Center (GLFHC) is a multi-site mission-driven non-profit organization employing over 700 staff whose primary focus is providing the highest quality patient care to residents throughout the Merrimack Valley. Nationally recognized as a leader in community medicine (family practice, pediatrics, internal medicine, and geriatrics), GLFHC has clinical sites throughout the service area and is the sponsoring organization for the Lawrence Family Medicine Residency program.
GLFHC is currently seeking a Community Support Services (CSS) Integrated Testing and Linkage to Services (ITLS) Coordinator. Under the supervision of the Community Based Manager, the CSS, Integrated Testing and Linkage to Services (ITLS) Program Coordinator is responsible for planning and coordinating mobile, stationary, and venue based outreach and groups, training of new /existent staff consistent with Massachusetts Department Public Hospital (MDPH) program guidelines, managing client data and reporting information, reviewing program data forms and client files, conducting prevention counseling and screening services to at-risk clients for HIV, Sexually Transmitted Infections (STI's) and Viral Hepatitis and completing other coordinator responsibilities as needed.
* Reviews program data forms and client files on a weekly basis and prepares the data for submission on a monthly basis to Manager for review.
* Monitors program's mailbox and MDPH Electronic Laboratory Reporting system (ELR) for the availability of test results for clients of the ITLS Program.
* Records test results in ITLS Client Log and files results into client charts for results delivery during the post-test counseling session
* Communicates with MDPH State Laboratory Institute Laboratory personnel about missing and/or inconclusive test results.
* Organizes and maintains proper documentation in order to prepare mandated reports under the direction of Community based manager PICSR Program Supervisor.
* Maintains statistics requires by funders, policy and/or regulation
* Assists in the preparation of community events including World AIDS Day, National HIV Testing Day etc.
* Assumes responsibility for ITLS administration in absence of PICSR Supervisor
* Provides prevention counseling and screening services to at-risk clients for HIV, Sexually Transmitted Infections (STI's) and viral Hepatitis.
* Collects and processes blood samples for HIV testing.
* Assists in the preparation and delivery of test results and documenting test results delivery.
* Maintains that all new phlebotomists are adequately oriented and that training is documented for the tasks required.
* Provides supported referrals to HIV positive clients for HIV medical care, case management services and other community resources.
* Provides supported referrals to clients testing positive for HCV and/or STIs to primary care for medical evaluation and treatment.
* Conduct onsite and community-based health education sessions on HIV/AIDS, STIs, viral Hepatitis and overdose prevention.
Qualifications
Experience
* Valid Massachusetts Driver's license and access to reliable transportation.
* Must have strong written and verbal communication, problem-solving, organization, accountability and program coordination skills
* Must be a passionate advocate for serving high risk populations (sexual or injecting partners of persons living with HIV/AIDS, Hispanic/Latino immigrant communities, other communities of color, the GLBT community (particularly men who have sex with men or transgendered persons), and/or high-risk heterosexuals.
Education
* Associates in Human Services or related field preferred; combination of education and experience will be considered.
* Bilingual, bicultural preferred (Spanish/English).
