Job Summary: In general, the Palliative Care Nurse Coordinator is responsible for screening palliative care referrals/ evaluating the appropriateness of palliative care referrals, gathering patient and family data in preparation for the palliative care consult by the palliative care team, coordinating palliative care consults, assessing member palliative are needs and monitoring the quality of outcomes. Essential Responsibilities: * Maintain the integrity of palliative care referrals: Screen, assess and prioritize palliative care consult referrals; Schedule and plan palliative care consults for the palliative care team while being sensitive to time constraints and resource.
