We are seeking a passionate and dynamic Counselling Coach/Case Manager to fill the role available as Community Program Manager. Offering flexible options for remote work, outreach, and in-office days. The role involves coordinating and running events, activity-based counseling and peer-led mentoring programs for those living with a disability and their families/carers. This unique role offers the chance to provide therapeutic support and case management to young people and adults living with disabilities, through a mix of one-on-one counselling, mentoring, and community-based activities. You’ll run and coordinate engaging programs such as horse riding, canoeing, gym sessions, wellbeing chats, and social events like movie nights and games nights. You will support peer leaders with lived experience, help them meet grant activity KPIs, and work alongside stakeholders, schools, clubs, and families to ensure inclusive and meaningful engagement. A key part of the role involves planning and facilitating group events, managing your own caseload, and helping to build capacity in others. You will be providing support to our NDIS and other clients directly in the form of outreach services and Group Education Programs. You will report to the CEO who will support remotely with the prioritising of roles and responsibilities, the training and support for this role and you develop into it, and supporting you meeting your KPI's. This role is instrumental within our organisation as it is at the forefront of all of our work with families across Queensland. The role can begin with a minimum of 3 days per week but we are ideally looking for someone with full time hours available. This will be a blend of intake, community engagement, program delivery, team leading and support, 1-1 counselling and coaching with clients, case management and administration for yourself and your direct reports.
This role is an advocacy role and is prefered that you have a lived experience of living with a disability that you can share with participants how you have navigated support services and opportunities. This will require you to run group Peer Programs with Adults and Young carers and other people living with a disability. To skill share, co-design and teach programs that help build participants capacity and confidence in advocating for themselves and speaking up.
