Overall Summary:
The Community Health Worker II (CHW II) at Bay Area Community Health will primarily work at serving and assisting our patients while located at different sites throughout the organization and community events. (CHW II) will work at different capacities based on their assigned area of training and specialty. The CHW II will be a trusted member of the community, and actively participate as a liaison between primary care, specialties and resources available to BACH patients. Activities may include, but are not limited to, outreach, engagement, care coordination, case management, linkage of the patient with resources at BACH and in the community such as food, housing, financial and transportation assistance and also linking the patient to care within the clinic.

Under the general direction of the supervisor/manager, the Community Health Worker performs a variety of services. Duties include:
CHW I tasks, plus:
Completion of CEC certification to determine eligibility and enroll patients in health plans
Analyze screening tool results and share with Care Team
Link patients to services within BACH and the broader community.
Participate in staff team meetings, case conferences and training.
Collect data and provides documentation in client files as required by agency and funders.
Participate in obtaining deliverables required for various grants, such as, but not limited to, eligibility assessment, socioeconomic needs assessment, and medical disparities.
Assist families with immediate crisis intervention, advocacy, problem-solving and other appropriate interventions, as needed
Utilize de-escalation techniques in situations as appropriate
Utilize program evaluation tools (i.e., pre/ post-tests, satisfaction surveys, etc.), analyzing and reporting on outcomes
Provide resources for various health topics, such as nutrition, physical activity, hygiene, sexual health, and substance abuse.
Maintain accurate and confidential records of interactions with students, families and other stakeholders.
Work flexible hours, which includes some evenings and weekends
Work at any of BACH location, including clinics, mobiles, schools, homeless encampments, warming centers and patients homes, or patient appointments as assigned
Secondary Responsibilities
Attends workshops, training, and meetings as needed, and as requested.
Performs other duties as assigned by supervisor.
Qualifications:
Required Education, Experience, and Training
High school diploma or equivalent
1-2 years of experience in a community-based health care service capacity preferred
Bilingual preferred
CHW Certification - Recertification and/or retraining process, as required
CEC Certification

Physical Requirements
This position requires:
Sitting, walking, and standing associated with office environment
Regular bending, stretching and lifting
Manual dexterity used to include, but not limited to, standard office equipment, and computer keyboard.
Traveling to work sites may include exposure to outside elements
Presenting information in front of a group or individual
Reading and writing in order to review records for accuracy
