This position is accountable for coordinating the implementation of PacificSource's community health strategies, programs, and partnerships. This role will support the development and execution of strategies related to social determinants of health (SDOH), health equity, healthcare workforce, tribal affairs, and Health-Related Social Needs (HRSN) by strengthening organization and community-wide efforts to address the unique needs of PacificSource members and communities. Essential Responsibilities: * Implement community health strategies, as informed by organizational strategic priorities and Community Health team objectives and initiatives. * Support the implementation of the HRSN program, its provider network, and the associated Community Capacity Building Funding grant process. * Support work led by PacificSource's Tribal Liaison to honor the sovereignty and self-determination of tribal nations in PacificSource Coordinated Care Organization service areas. * Support implementation and required reporting of healthcare workforce strategies in communities served. * Coordinate internal and external stakeholders utilizing the Community Information Exchange software platform and other efforts used to support SDOH strategies. * Implement health equity strategies in Medicaid, Medicare, Dual Eligible Special Needs Plans, and commercial plans, as applicable. * Support community engagement efforts related to Community Health, Medicaid Administration, and other internal team projects. * Provide project support and coordination for initiatives and priorities of the Community Health team and cross-functionally, as needed. * Coordinate execution of necessary contractual and/or regulatory agreements required to accomplish community health objectives. * Prepare materials for use at multi-stakeholder boards, steering committees, and work groups as needed by team members to represent the organization and its programs. Supporting Responsibilities: * Participate in internal and community-based presentations. * Meet department and company performance and attendance expectations. * Follow the PacificSource privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information. * Perform other duties as assigned. SUCCESS PROFILE Work Experience: Minimum of three (3) years of experience in healthcare, community health, provider organization, or related area. Relevant experience the following areas: managed care, community health, strategy implementation. Experience in managing complex projects or project management certification is desirable. Education, Certificates, Licenses: High School diploma required. Bachelor's degree in health care related field or equivalent years of additional relevant experience in lieu of degree preferred. Bi-lingual in English/Spanish highly preferred. Knowledge: Excellent public relations, presentation, and interpersonal skills. Demonstrated successful communication skills, including public presentation, training, meeting facilitation. Maintain high level of knowledge of company products, health reform trends at the Federal and State levels, Demonstrated skills with Microsoft Office software products. Knowledge of state and federal regulators and basic health plan information. Program and project management skills. Travel is required approximately 20% of the time.
