Job Overview

The Community Health Program Coordinator is responsible for identifying high-risk patients with poor health outcomes. The role is responsible for program activities such as referral coordination, eligibility checks, , and monitoring CHW caseloads. The role will also support with data collection and analysis for overall program evaluation. Our organization offers an innovative, evidence-based health care program that improves health outcomes for low-income, high-risk patients using Community Health Workers (CHWs). In this model, CHWs provide structured advocacy, social support, and navigation to high-risk patients in order to help them reach their health goals.

Minimum Requirements

  • High School Diploma required.
  • Minimum of 3 year experience with data collection, management, and/or analysis.
  • Minimum of 2 year experience extracting information and exporting data to other applications.
  • Experience with Epic EMR system a plus.

Compensation

$24.00 - $28.15 hourly
