Salary:

$28.60/ hour- $30.7/ hour

Summary:

The Community Case Manager (CCM) will support the San Francisco Drop In Service Center by handling the office flow, engaging and following up with clients, completing enrollments, referrals, advocacy, data entry/ management, resource navigation, crisis de-escalation, socialization, and community building. The CCM works in a fast-paced environment that seeks to serve all veterans and is comfortable engaging with veterans who may be intoxicated and/or presenting with active mental health symptoms.

Requirements and Responsibilities:

  • Engage with veterans to screen and triage access to services at Swords to Plowshares; Provide information on available internal and external resources and support as front desk reception

· Completes enrollment into Swords to Plowshares electronic case management platform (ECM) and makes internal and external referrals to resources for all eligible veterans.

  • Provide a safe and supportive environment for veterans seeking services with an understanding of cultural humility, harm reduction, and trauma informed care practices, ensuring the quality of services and data entry follow Swords to Plowshares and the various funders policies, procedures, and standards.
  1. · Follows up with clients over the phone and in person for wellness checks and referral status updates
  2. · Efficient and accurate data entry, updates, notation and documentation uploading in ECM
  • Close collaboration with other internal departments on the coordination of client needs, data entry, data reporting, and client activities
  • Perform client wellness checks in person at the Drop In and over the phone
  • Assist clients with paperwork and requests for documents, birth certificates, ID, income verification, etc.
  • Assist with daily operations of Drop In Service Center.
  • Perform other tasks as delegated by the San Francisco Service Center Manager
  • Must be able to carry out the essential duties of the position within what can be considered reasonable accommodation

Education:

  1. · High School diploma or GED required.
  2. · BA/BS desired but equivalent professional experience is acceptable.

Experience:

  1. · Minimum of two (2) years’ experience providing case management and/or working with complex behavioral health and substance use challenges in a fast-paced social service office setting.
  2. · Translatable military experience and skills may be alternatively substituted.

Qualifications and Skills

  • Ability to work in a team environment and communicate professionally with appropriate staff (internal and external) to coordinate client support and provide updates.
  • Ability to appropriately respond to conflict and/or emergency situations and utilize de-escalation in support of a resolution.
  • Working knowledge of local social service resources, trauma informed care practices and the harm reduction model
  • Excellent customer service skills
  • Excellent organizational and time management skills
  • Flexibility and willingness to perform multiple jobs in a work environment with changing priorities
  • Must be able to set appropriate boundaries
  • Proactive and collaborative problem-solver
  • Detail-oriented
  • Demonstrated ability to interact comfortably with people from all walks of life
  • Ability to follow through on assigned tasks in a timely manner
  • Excellent written and oral communication skills
  • Must possess computer proficiency in MS Office Suite, general web-based applications and Internet search engines

Compensation is based on a competitive public interest salary scale. Swords to Plowshares offers a generous benefits package:
