Summary

Description

The Comin’ Up Case Manager is responsible for case management, programming, administrative compliance, facility and child safety, relationships, and transportation of members. Comin’ Up Gang Intervention Program serves gang-involved youth between the ages of 12-18 years in Fort Worth communities. The goal of the Comin’ Up Gang Intervention Program is to positively impact the lives of at-risk and gang-involved youth by providing needs-based services and activities to reduce the level of gang violence in the City of Fort Worth.

Case Management

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Provide holistic and comprehensive case management services to members at assigned site(s), including but not limited to intake, assessment, goal setting, service planning, progress monitoring, follow-up services, advocacy, documentation, and referral services

Comin Up Programming

  • Accountable for assigned programming and activities
  • Accountable for quality programming and activities
  • Accountable for promoting site participation
  • Actively participate in a multi-disciplinary team
  • Document all services and contacts
  • Accountable for accurate and timely case notes, data entry, and reports

Administrative Compliance

  • Complete all required reports and administrative duties on time.
  • Collect and manage required data and internal and external reporting. Ensure accuracy of data and reporting. Ensure the effective completion of all assessment and measurement activities of programs
  • Accountable for the accurate and timely collection and review of membership applications
  • Comply with all administrative policies and procedures, processes, and controls
  • Accountable for accurate and timely paperwork, reports, and data entry
  • Accountable for utilizing all BGCGTC communication tools regularly and effectively

Facility And Child Safety

  • Accountable for adherence to all organizational health and safety policies and implementation of all safety practices and procedures at assigned work location(s)
  • Take all necessary and reasonable precautions to protect members, staff, equipment, materials, and facilities. Abide by facility maintenance protocols and procedures to ensure compliance
  • Accountable for reporting incidents and completing documentation within the required timeline
  • Maintain discipline, de-escalate conflict between members and/or others
  • Obtain and renew CPR and First Aid certification by the deadline

  • Be a positive role model
  • Maintain emotional control and make good decisions under pressure
  • Represent the Boys & Girls Club in a positive manner
  • Actively build and maintain strong, positive relationships with both internal and external partners
  • Build and nurture rapport by actively engaging with youth in a respectful and empathetic manner.

Transportation Of Members

  • Transport youth participants in compliance with safety guidelines to and from different services
  • Accountable for operating vehicles in a safe and efficient manner
  • Accountable for reporting incidents and completing documentation within required timelines
  • Maintain discipline, and de-escalate conflict between members and/or others

Attendance

  • Regular attendance is required; arrive on time, work required hours/shift, and leave at the scheduled time

Nonessential Duties

  • Complete all other duties as assigned

Requirements

Education and Work Experience Requirements:

  • Bachelor’s Degree from an accredited college or university, or equivalent experience
  • Minimum of one year of experience working with at-risk youth and/or gang-involved youth
  • Minimum of 6 months of case management-related experience

Minimum Qualifications Requirements

  • Ability to work regular program hours, which include day and evening, with occasional weekends
  • Ability to assist youth to progress and achieve their individual goals
  • Ability to establish rapport and build relationships
  • Ability to persuade, influence, and motivate others into action
  • Ability to identify needs and find solutions
  • Ability to work with at-risk youth
  • Knowledge of case management techniques and/or processes
  • Knowledge of conflict resolution or mediation techniques
  • Knowledge of problems and barriers at-risk youth face
  • Knowledge of technology and software such as tablets, social media, cloud-based tools, virtual meeting tools
  • Must be a team player, willing to collaborate, and make team decisions
  • Must be able to obtain CPR and First Aid certification
  • Must be able to pass agency background screens and drug screens at any time
  • Must have reliable transportation, liability insurance, a satisfactory driving record and a valid driver’s license for operation of a motor vehicle relevant to carrying out duties

Preferred Qualifications

  • Bilingual
  • Work experience with or program knowledge of the Boys & Girls Club Movement (national and/or local Club) is an advantage

  • Pay: $19.78 per hour
