Paving the Way Multi-Service Institute is excited to announce an opportunity for a
Clinical Case Manager to join our dedicated team. In this role, you will provide essential support to clients facing various life challenges, ensuring they receive the necessary resources and interventions for their mental health and wellbeing. As a Clinical Case Manager, you will act as a liaison between clients and service providers, conducting assessments, developing treatment plans, and facilitating access to community resources, thereby empowering clients to achieve their individual goals.

Key Responsibilities:
- Client Assessment: Manage a caseload and conduct thorough assessments of clients' needs, preferences, and goals to develop individualized treatment plans that align with their unique circumstances.
- Care Coordination: Act as a central point of contact for clients, coordinating care among various service providers to ensure holistic and effective treatment.
- Resource Navigation: Assist clients in accessing community resources, government benefits, and other support services necessary for their recovery.
- Documentation & Reporting: Maintain accurate and timely documentation of client interactions, case notes, and progress reports in compliance with agency standards.
- Advocacy & Support: Advocate on behalf of clients to facilitate communication with external resources and services, ensuring their needs are met and fostering a culture of empowerment.
Skills & Qualifications:
- Education: Master’s degree in Social Work, Psychology, or related field;
- Licensure: Current licensure (LGSW, LGPC, or equivalent) is required.
- Experience: 1-4 years of experience in case management or a similar role, preferably within mental health or social services.
- Skills: Strong communication, organizational, and analytical skills. Proficiency in electronic health records and an understanding of community resources.
- Preferred: Demonstrated ability to work independently as well as collaborate with multidisciplinary teams.
