Job Status: Mon-Fri 8am-4:30pm

Position Summary

The Community Intervention Team (CIT) is a community-based case management program that serves adults with mental health and/or substance use disorders, supporting them with community living and independent living skills. The Clinical Case Manager will work with the Lead Case Manager and Community Case Management Supervisor to provide person-centered, trauma-informed care to a caseload of approximately 15-17 individuals and facilitate wraparound services to provide intensive and high quality care. Areas of focus may include symptom management, medication monitoring, housing, medical care, psychiatric care, crisis management, and benefits coordination.

Essential Functions

  • Engage clients with community resources such as food pantry, healthcare providers, employment, crisis services, benefits, etc.
  • Conduct screenings and assessments to evaluate individual need and provide tailored support to each individual on caseload
  • Facilitate wraparound recovery plan meetings every 6 months with client and other service providers
  • Assist clients in creating goals and implementing action plans to achieve them
  • Document all client interactions by entering progress notes into EHR
  • Work within a multi-disciplinary team including other case managers, RN, medical case manager, therapist, and psychiatrist
  • Providing transportation via company vehicle to essential appointments regarding healthcare, legal, housing, etc.
  • Managing schedule independently and managing time to allow for client care and paperwork needs
  • A Master’s Degree in health/social services, preferred
  • 2+ years of experience in Human Services field, required
  • A Bachelor’s Degree or higher in health/social services, required
  • SAC or SAC-IT, preferred
  • CCS experience and certified, preferred
  • Bi-lingual, preferred
  • Valid Driver's License
