Job Summary

The Recovery Mobile Clinic is a 501c3 nonprofit organization that operates in RV units and partners with community agencies to bridge the gaps in healthcare access. We travel to homeless shelters, transitional houses, community centers, churches and more. The Aim is to bridge the gaps in healthcare and to improve the collaboration of agencies.

We are looking for a qualified Case Manager to join our team! You will play a crucial role in assisting patients in coordinating their care patients and implementing community engagement plans in collaboration with patients’ provider.

Operating with professional expertise and deep care for patients, you are a natural problem solver and self-starter. You enjoy working in fast-paced environments that afford you the autonomy to bring your best.

Responsibilities

• Implement standards of care for Case Management services
• Participate in patients’ plans of care.
• Coordinate outpatient appointmetns with primary care physicians, psychiatry and other specialties. Assist clients with setting up mediacid rides and other transporation access.
• Assist patient s with Medicaid Apps and SNAP benefits
• Make follow-up calls to assess and continue the plans of care
• Assist with Data collection in out new STI/HIV Services
• Maintain accurate and up-to-date records
  
  
  

Qualifications

• Associate degree in Psychology or Social Work
• Valid driver’s license
• Psych or social work of community health experience is a plus
