Work with various providers in the region or system to facilitate access to services. Is well versed in resources available in the region. Supports the No Wrong Door philosophy.
Facilitate community appointments and meetings to assist clients with achieving their goals. This includes coordinating with community supports, transporting clients to and from scheduled appointments, and being an advocate for clients self-determination.
Education: Bachelor’s degree in a mental health-related field such as Psychology, Social Work, or Counseling required
