Position Summary:
The Case Manager provides supportive services to high-risk individuals through assessments, individualized planning, and service coordination. This role ensures participants access to housing, employment, behavioral health, and trauma recovery support.
Job Responsibilities:
- Complete intakes and eligibility documentation.
- Develop and implement individual case plans with measurable goals.
- Maintain a caseload of up to 25 participants.
- Provide referrals to behavioral health, housing, job training, and legal resources.
- Track outcomes and update participant files with progress notes.
- Attend interdisciplinary team meetings and case conferences.
- All other duties as assigned.
Education, Experience, and Skills Required:
Education: Bachelor’s degree in Social Work, Psychology, Human Services, or related field.
Experience: Two years of experience in case management or violence prevention work.
Skills:
- Strong documentation and time management skills.
- Effective communication and interpersonal abilities.
- Trauma-informed and culturally competent approach.
Location:
In-person/On-site; both in IL and MO
