Summary

Program and Role Description:

Parents by Choice’s Preventative Wraparound Service Program (Pre-Wrap) encompasses a wide range of

support services strategically coordinated and designed to address the unique and changing needs of

children and families at risk of experiencing out of home placement. The Wraparound approach ensures

that families facing complex and multi-dimensional challenges receive personalized, comprehensive,

family-driven, and strengths-based team services within their community and system(s) of care. Our

interventions are designed to keep children with their families and in their communities.

Pre-Wrap Case Managers are responsible for helping the family, and their team, create a customized

plan, implement strategies of the plan, and guide families through the system of care. Pre-Wrap Case

Managers are essential to the coordination and facilitation of the support provided to families,

ensuring collaboration among team members and effective communication.


Qualifications:

  • An Associate of Arts degree in a discipline related to social services is preferred.
  • Knowledge of Foundational Wraparound services and Child/Family Team Meetings (CFT’s)
  • Bilingual in Spanish and English is preferred.
  • Experience in working with children and families in a professional setting.
  • Valid California Driver’s license, proof of auto insurance and a driving record acceptable to the Agency’s insurance carrier.
  • Health clearance.
  • Fingerprint and Child Abuse Index clearance from the Department of Justice.
  • Meets all state requirements as set by Community Care Licensing pertaining to the hiring of employees by a Licensed Community Care Facility.


Responsibilities and Expectations:

Preventative Wraparound Case Managers shall be responsible for, but not limited to, the following duties:

  • Pre-Wrap Case Managers are responsible for helping the family and their team create a customized plan, implementing this plan, and guide families through the system of care. 
  • Effectively maintaining a caseload of up to 20 families.
  • Facilitate regular team meetings and ensure effective communication among family team members.
  • Provides on-going support to families, guiding and encouraging them as they complete the program.
  • Provide observation of parent/child interactions, according to program requirements and parent needs.
  • Conduct and document contact with clients. Provide monthly updates of client progress to their assigned social worker. Keep adequate records of these visits on file per agency standards.
  • Ability to communicate effectively with program partners and stakeholders.
  • Act in compliance with Agency’s policy on communication—emergency and otherwise. Respond in an appropriate and timely manner to parents, staff, and others.
  • Provide support to other staff members as needed.
  • Accurately report incidents, issues, and concerns to the supervisor in a timely fashion.
  • Meet weekly for supervision and discussion of program participants and activities.
  • Maintain professional working relationships with program participants and others.
  • Educate yourself on State and Agency regulations.
  • Complete own administrative paperwork (time sheets, mileage, time off requests, expense reimbursements, etc.) on time.
  • Demonstrate sensitivity and responsiveness to cultural differences.
  • Represent Agency and self in a professional manner.
  • Maintain appropriate professional boundaries with staff, families, and community resources.
  • Periodically assists with agency events and outreach activities.
  • Attend Agency staff meetings and training sessions.
  • Drives own personal vehicle on the job to accomplish job duties.
  • Perform additional duties as assigned by and in support of other staff.
  • Contribute to the agency’s performance and quality improvement by identifying areas of improvement in service delivery and submitting recommendations for improvement.
  • Contribute to improve outcomes and service delivery for all service recipients.


Physical Demands:

To perform the job duties associated with this position, the employee is regularly required to sit for long

periods of time, stand, walk, use hands to finger, handle, or feel; reach with hands and arms; talk and

hear. The employee is occasionally required to stoop, kneel, crouch or crawl; lift and/or move up to 25

pounds. Visual ability to judge distance, color, focus, and see peripheral objects is also necessary.


Professional Growth and Development:

Parents by Choice employees attend a combination of agency and professional workshops, including

conferences to expand skills and their knowledge base. Parents by Choice provides ongoing training

opportunities for all-staff members each year, each employee is responsible to attend these training

sessions.
