We're looking for a Case Manager for our Comprehensive Community Treatment Program(CCTP) who is an empathetic individual dedicated to providing quality support and care to others. Qualifications:
* Must have bachelor's degree or master's degree in psychology, social work or related field and at least (1) year experience.
* Preferred experience with multicultural backgrounds and language capacity in Spanish or Haitian Creole.
* Must have humility in diversity and history of direct work with BIPOC, Hispanic/LatinX and LGBTQ* individuals.
* Must have a strong commitment to recovery-oriented services.
* Required to have a valid MA driver's license and reliable transportation.
Responsibilities:
* Provide side by side interventions to individuals in the community and in client's residence based on client preference, client need, and at the clinical direction of the Team that focus on problem solving, skills training, modeling behavior, optimism and encouragement.
* Assist the team and individuals in providing on-going assessment of symptoms and responses to staff interventions.
* Participate in the development of treatment plans for each individual in which you are a member of their treatment team. Ensure that treatment goals are culturally competent, strength based, realistic, attainable and promote recovery.
* Ensure that each individual is actively involved in their treatment planning process and their preferences are embraced and collaborate daily or as needed on treatment plan updates with other team members.
* Provide interventions to individuals as stated by their current treatment plan or in response to the daily needs of individuals per supervisory direction and/or through the Team Meeting process.
* Provide culturally sensitive and targeted case management services to families and individuals as is determined by the team clinicians, supervisors and Director.
* Provide case management supports to individuals including: managing income, insurance, other entitlements, accessing other services and community resources and scheduling/coordination of all medical and psychiatric appointments.
* Provide billable and non-billable services through CCTP/Outpatient in accordance with Eliot and CCTP/Outpatient policies including case management (CSP) billing, and services within billing procedural scope
* Promote concepts of health and wellness to assist individuals with accessing information and services related to medical care, nutrition, exercise and smoking cessation as appropriate. To assist individuals with accessing community resources that support these health related goals.
* Act as on call support to individuals on a rotating basis. Assist with issues pertaining to individual care, medications, psychiatric/medical crises support for individuals. Contact the supervisor immediately for any support as needed.
* Perform other job-related duties as assigned.
Salary: $50,000.00 annually
Schedule: Monday-Friday, 9am-5pm
