Job Summary

Responsible for providing support to Case Managers to ensure case management services, communications, and appropriate documentation is consistently processed, while serving as a liaison for patients, family of patients, and hospital staff to ensure patient satisfaction, experience, and quality of services are delivered.

Required Qualifications

  • Require a High School Diploma, GED Equivalent, or Certificate, associate degree in related field preferred.
  • Require a minimum of two (2) years of customer service experience in a hospital or related setting, preferably in inpatient rehabilitation.

Preferred Qualifications

  • Prefer basic knowledge of computer systems.
  • Prefer demonstrated ability in establishing and maintaining effective working relationships.
  • Prefer demonstrated initiative and self-direction.
  • Prefer effective oral and written communication skills.
  • Prefer previous healthcare experience with discharge planning activities.
