Responsible for providing support to Case Managers to ensure case management services, communications, and appropriate documentation is consistently processed, while serving as a liaison for patients, family of patients, and hospital staff to ensure patient satisfaction, experience, and quality of services are delivered.
Required Qualifications
Require a High School Diploma, GED Equivalent, or Certificate, associate degree in related field preferred.
Require a minimum of two (2) years of customer service experience in a hospital or related setting, preferably in inpatient rehabilitation.
Preferred Qualifications
Prefer basic knowledge of computer systems.
Prefer demonstrated ability in establishing and maintaining effective working relationships.
Prefer demonstrated initiative and self-direction.
Prefer effective oral and written communication skills.
Prefer previous healthcare experience with discharge planning activities.
