About The Role

We’re looking for a highly organized professional to be our “go to” for the daily unit operations of discharge planning and case management.

  • Ideally have extensive case management knowledge
  • Familiar with the resources in the community
  • Review all documentation before admittance.
  • Lead discharge planning meetings
  • Attend Treatment meetings

Your main responsibility will be to the people we serve, ensuring that policies and procedures are followed, and that benchmarks for performance are met in an efficient and effective manner. As such, you’ll have a strong relationship building with both internal and external providers and resources.

Our Team Lead provides recovery-oriented support to individuals with Serious Mental Illness transitioning from inpatient psychiatric hospitalization to our program before reintegrating back to community living situations. We’re looking for people who have experience developing and mentoring a team, who believe in a strengths-based approach, and use positive practices to help guide their everyday activities. The Extended Acute Care is a licensed, 16-bed temporary residential treatment program

Must have a master's degree in human services AND one-year experience working with people with serious mental illness diagnoses OR bachelor's degree in human services AND three-years of experience working with people with serious mental illness diagnoses Licensed preferred

  • Must pass a bi-annual physical
  • Must have a valid PA driver’s license and be able to drive an organization automobile/15-passenger van
  • Must have Act 33/34/73 Clearances upon hire

Work Schedule Monday-Friday 8am-4pm
