This is a work from home flexible position with 25-50% travel in Passaic County New Jersey. Travel will be up to a 50 mile radius from your home. Standard Working hours Monday – Friday 8-5 pm. The ICM Case Manager RN helps elevate patient care by serving dual eligible populations enrolled in Medicare and Medicaid with complex health and social challenges. Responsibilities include assessing, planning, implementing, and coordinating case management activities to evaluate medical needs and facilitate overall wellness. The role involves using clinical tools to evaluate member needs, applying clinical judgment to reduce risk factors, conducting assessments considering multiple diagnoses, collaborating with healthcare stakeholders, and utilizing case management processes in compliance with policies. Motivational interviewing skills are used to engage members. Security measures include meeting in public places or security escort if needed. Required qualifications include active RN licensure in New Jersey, 3+ years clinical experience, willingness to travel 25-50% in Passaic County, and residing close to or within Passaic County. Preferred qualifications include Certified Case Manager, 2+ years care management experience, bilingual skills, independent work capability, and proficiency with corporate software. Education required is Associate's Degree in Nursing; Bachelor's preferred. Weekly hours are 40, full-time.
