SUMMARY: 

The Care Manager is responsible for collaborating with patients and their families, contracted hospitals, preferred providers, and the Compass Complex Discharge Solutions (CCDS) interdisciplinary team to develop and execute an efficient and safe patient discharge plan in accordance with all CCDS policies and procedures. The Care Manager will provide all services to patients with a patient-centered approach that aligns with CCDS’s core values.



ESSENTIAL FUNCTIONS:

·      Use health information technology to facilitate collaboration with a hospital’s case management team and CCDS’s interdisciplinary team to complete comprehensive patient assessments and discharge plans.

 

·      Develop and maintain a patient-centered discharge plan that coordinates and integrates a patient’s clinical and non-clinical healthcare related needs and services, while complying rigorously with CCDS processes.

 

·      Monitor and facilitate implementation of and provide updates to the discharge plan as needed.

 

·      Engage families and hospital Case Management team in the discharge planning process.

 

·      Secure all health records and other protected information with the highest regard to confidentiality and HIPAA laws and regulations.

 

·      Use health information technology to update patient records log daily.

 

·      Aid in gathering forms, documents, statements for Medicaid/ICP Applications.

 

·      Facilitate implementation of Guardians and/or Power of Attorneys as needed.

 

·      Assist in gaining access to different health facilities for patient referrals.

 

·      Facilitate patient placement with preferred providers.

 

·      Facilitate transportation for patient discharge to accepting facility.

 

·      Provide a discharge summary to accepting facility staff.

 

·      Maintain compliance with all state and federal laws and regulations, Medicaid compliance, and CCDS policies and procedures.

 

·      Document all services and maintain appropriate records following all established documentation procedures.

 

·      Participate in opportunities for continued training and education.

 

·      Perform all other duties relevant to the position as assigned.


KNOWLEDGE, SKILLS, AND ABILITIES:

  • Basic knowledge of clinical and psychosocial aspects of patient care.
  • Ability to conduct thorough, accurate patient assessments.
  • Basic technology and data entry skills and understanding of health records.
  • Knowledge of HIPAA and all applicable laws and regulations and patient confidentiality requirements.
  • Knowledge of ethical and professional responsibilities and boundaries.
  • Demonstrate professional work habits including dependability, time management, independence and adherence to deadlines.


EDUCATION AND EXPERIENCE:

·      Bachelor’s degree in a healthcare-related field with a minimum of two years of Complex Discharge experience, OR

·      A license as a Registered Nurse or Social Worker with a minimum of two years of Complex Discharge experience, which can include any employment experience and is not limited to case management / service coordination duties


 PHYSICAL REQUIREMENTS:

  • Ability to sit/stand throughout day to accomplish job.
  • Ability to electronically enter data, notes, and other documentation into a computer and/or tablet.
  • Must be able to travel to partner hospitals as needed.

·       Protect the privacy of all patient information in accordance with CCDS’ privacy policy, guidelines, and practices as required by federal and state law, and in accordance with general principles of professionalism as a health care-related organization.

 

·       May access protected health information and other patient/health information only to the extent that is necessary to complete your job duties; you may only share information with those who have a need to know specific information you have in your possession to complete their job responsibilities related to company operations.

 

·       Report any concerns regarding CCDS policies and guidelines on patient privacy and any observed practices in violation of that policy to the CEO.

 

·       Actively participate in CCDS’s privacy training and communicate privacy policy information to coworkers and others in accordance with company policy.
