Program overview: Raising Strong Families (RSF) is an innovative pilot project developed by the Coos Coalition to help the most vulnerable families in the North Country. We take a proactive, family-centered approach, focusing on building a dedicated team for each family. This team works to cut through bureaucratic red tape, reduce service redundancy, and align strategies across agencies, providing tailored, long-term support. At its core, RSF is driven by values like being parent-centered and family-driven, addressing core needs first, engaging genuinely, using plain language, and partnering with the community. These guiding principles ensure that families receive the respectful, individualized support they deserve. RSF is crucial in addressing health disparities and promoting equity within communities. By connecting families with essential resources and services, it helps alleviate barriers to health and well-being, contributing to healthier, more resilient communities. As a Care Coordinator, you will: Develop trusting relationships with families through strength-based, family-centered interventions. Assist families in setting and achieving their goals using evidence-based practices. Conduct routine screenings, provide referrals, and maintain accurate case records. Link families to both formal and informal community supports. Advocate for families while collaborating with team members and stakeholders. Facilitate wraparound team meetings to ensure collaborative support. Stay informed about local, state, and national resources. Participate in ongoing professional development and training. Represent the Family Resource Center in various settings.
