I. Position Function

Under supervision, serves as a liaison between the physicians, office support staff, and the Practice Manager. The Care Coordinator is responsible for assisting the Practice with day-to-day operational and administrative aspects of the physicians’ offices. The main responsibilities include performing prior authorizations for medications for patients along with reviewing and submitting PT-1 forms for all Medicaid patients of the clinic. Serves as immediate contact for all internal and external customers. Works independently to resolve issues in a timely manner. Interacts with individuals at all levels within the office and the organization.

II. Job Relationships

The Office Coordinator reports directly to the Practice Manager but interacts administratively with the clinical staff and administrative staff.

IV. A. Responsibilities/Essential Functions

"Provides superior customer service to internal and external clients, customers, and patients as referenced in the Service Excellence Standards."

Performs duties and provides assistance according to Faculty Practice Plan policy and procedures, as well as any directives from the Office Manager.

  • Ensures efficiency and productivity with respect to the administrative and clinical functions of the office. Daily monitoring of physician and patient activity in the practice; allocates resources to necessary tasks and set priorities
  • Performs PT-1 forms and necessary forms for Medicaid patients.
  • Assists clinical staff with prior authorization for medications as requested
  • Assists with interpretation for patients and staff
  • Checks voicemails from patients and triages appropriately
  • Assurance that all “Best Practice” initiatives are followed.
  • Maintains patient confidentiality according to CPN and hospital standards.
  • All other duties as assigned

B. Responsibilities/Non-Essential Functions

V. Reporting Requirements:

Reports on trends in adverse patient occurrences as soon as detected to the physician and the Office Manager. Reports to the physicians and the Office Manager any quality issues requiring consideration or action.

VI. Accountability

Shall be accountable for maintaining the confidentiality and security of all Practice Plan-related and patient-related data and information.

Shall be accountable for abiding by all relevant Faculty Practice Plan policies and procedures.

VII. Qualifications

Minimum Education:

HS Diploma, associate or bachelor’s Degree preferred, but not required. Relevant work experience may substitute for degree requirements.

Minimum Experience

Must have 2-3 years experience in a healthcare environment.

Minimum Skills/abilities

Must have excellent written and verbal communication skills, and a strong desire to work as part of a team Must be able to multi-task and prioritize daily activities

Proficiency with computers and window-based products

Knowledge of insurance coverage systems, including but not limited to co-payments, referrals, HMO, PPO and capitated products, preferred

Experience With Organization Of Medical Charts Preferred

Ability to analyze operational issues and solve them creatively

Strong orientation to patient care in accordance with the Faculty Practice Plan's values.

Training

Training in the courses of Quality Improvement, Performance Improvement and other educational programs are encouraged.

Special Qualifications

Capacity to analyze, to think creatively, and to weigh alternatives. Perception of people and an awareness to deal with conflict successfully and attain conflict resolution.
