Job Description

The Case Manager – IOP/Psych is responsible for delivering group and 1:1 therapy, social services, including psychosocial assessments, advocacy, teaching, referrals to community services, and assistance. Interviews patients, makes assessments, and interacts with families, significant others, physicians, and staff, to ensure the highest quality care and service to provide emotionally and cognitively impaired individuals the ability to identify problem areas and promote goal attainment using structured activities. Ensures documentation in the medical record is clear and complete.

Responsibilities

Interviews patients and significant others to obtain information about social, emotional, and family situation. Communicates social and family information to attending physicians and other patient care team members. Enables patients and their significant others to understand, accept and follow medical recommendations through individual or group therapy or group conferences. Intervenes appropriately in instances of abuse, assault or neglect, or when adequate financial or social resources are not available. Makes appropriate referrals to resolve patient issues effectively.|Participates as a team member in conferences on behalf of patients with other health care personnel and other community agencies. Contacts community resources and other pertinent sources in order to arrange services on patient’s behalf|Provides crisis management and ongoing assessment of client functioning, including assessment of and suicidal or homicidal ideation as needed. Documents appropriately on the progress of clients: group notes, progress notes, and treatment plans. Ensures HIPAA compliance.|Empathizes with patient and patient support systems, expresses compassion and understanding for the situation and demonstrates care which is appropriate for the age, and cultural, religious, gender identity, or ethnic background. Engages in caring behaviors, taking conscious ownership for work and takes deliberate/thoughtful actions regarding how to best carry out interactions with the patient, families, physicians, and co-workers.|Ensures department performance, quality, and process improvement. Oversees Social Services quality control policies; documents all quality control activities, instrument and procedural calibrations, and maintenance performed. Appropriately educates and mentors. Demonstrates problem-solving, critical thinking, and prioritization of work to resolve issues as they arise in an appropriate and timely manner.

Qualifications

Ability to provide group and 1:1 therapy. Computer Proficiency and Literacy required. Knowledge of CMS, Medicare/Medi-Cal required. Familiarity of Joint Commission, CMS, CDPH requirements required. Excellent written and verbal communication skills in English. Ability to work in a fast-paced multidisciplinary environment. Strong time management skills and ability to effectively prioritize task. Ability to establish and maintain effective working relationships across the organization. Two years of experience in hospital setting preferred. MFT intern or LMFT, ASW OR LCSW preferred. Knowledge of discharge planning and social service practices preferred.Master’s degree in social work, Psychology or related degree required.AHA Basic Life Support required. De-escalation certification upon hire required. Hospital Fire and Life Safety Card (Los Angeles City Employees only) required. Current Licensure as a Registered Nurse required.
