Position Overview

We are seeking a dedicated full time Case Manager to join our team. This role is ideal for someone passionate about helping others, with experience in behavioral health and substance abuse treatment. You’ll play a key role in coordinating care, facilitating groups, and supporting clients through every stage of their recovery.

Key Responsibilities

  • - Conduct client intakes and assessments
  • - Develop and manage individualized service plans
  • - Assist with FMLA and Short-Term Disability documentation
  • - Coordinate medical, psychiatric, and legal appointments
  • - Facilitate group sessions and provide direct client support
  • - Monitor client progress and update treatment plans
  • - Maintain accurate documentation in KIPU EMR within 24 hours
  • - Collaborate with healthcare providers, community partners, and community agencies
  • - Participate in weekly interdisciplinary team meetings and supervision
  • - Ensure compliance with ADHS and Joint Commission requirements and company policies
  • - Build and maintain professional relationships with referring agencies


Qualifications

  • - High School Diploma required; Bachelor’s or Master’s degree preferred
  • - Minimum 1+ year of case management experience
  • - At least 1 year of experience in substance abuse or behavioral health
  • - Valid driver’s license and reliable transportation
  • - AZDPS Level 1 Fingerprint Clearance Card
  • - Current CPR/First Aid certification
  • - Preferred: PRSS certification and behavioral health coursework

Physical Requirements

  • - Ability to walk, bend, stand, and climb stairs
  • - Lift up to 25 lbs occasionally
  • - Communicate effectively in English
