Position Overview
We are seeking a dedicated full time Case Manager to join our team. This role is ideal for someone passionate about helping others, with experience in behavioral health and substance abuse treatment. You’ll play a key role in coordinating care, facilitating groups, and supporting clients through every stage of their recovery.
Key Responsibilities
- Conduct client intakes and assessments
- Develop and manage individualized service plans
- Assist with FMLA and Short-Term Disability documentation
- Coordinate medical, psychiatric, and legal appointments
- Facilitate group sessions and provide direct client support
- Monitor client progress and update treatment plans
- Maintain accurate documentation in KIPU EMR within 24 hours
- Collaborate with healthcare providers, community partners, and community agencies
- Participate in weekly interdisciplinary team meetings and supervision
- Ensure compliance with ADHS and Joint Commission requirements and company policies
- Build and maintain professional relationships with referring agencies
Qualifications
- High School Diploma required; Bachelor’s or Master’s degree preferred
- Minimum 1+ year of case management experience
- At least 1 year of experience in substance abuse or behavioral health
- Valid driver’s license and reliable transportation
- AZDPS Level 1 Fingerprint Clearance Card
- Current CPR/First Aid certification
- Preferred: PRSS certification and behavioral health coursework

Physical Requirements
- Ability to walk, bend, stand, and climb stairs
- Lift up to 25 lbs occasionally
