Make an Impact! Coordinating the care of a patient’s needs requires knowledge, communication, and compassion! Make a difference for patients and their families as a Registered Nurse Ambulatory Care Coordinator. You will monitor outcomes and utilization of services, while maintaining continuity of care and communication with providers, practice staff, and payers. In addition, you utilize age appropriate clinical assessment and intervention with patients to support their psycho-social and medical needs of patients and families. The care of patients does not stop in the office: as a RN Ambulatory Care Coordinator, you will connect with eligible patients for initial and periodic telephonic outreach calls focusing on clinical assessments, early symptom recognition and intervention, self-management abilities, and health literacy.

Position: Registered Nurse RN - Ambulatory Care Coordinator
Shift: Days, no weekend, no holidays (Hybrid)
Experience
Minimum of two years recent experience in telephonic care management outreach required.
Minimum of two or more years of recent home care experience preferred
Minimum of two years recent insurance, case management experience preferred.
Education
Bachelor’s Degree Preferred.
Licensures/Certifications
RN License Required.
