Summary

Admissions Coordinator:

The ideal candidate will be a dynamic self-starter with at least two years of experience in a skilled nursing facility and will have excellent communication and organizational skills. We are a large skilled facility, with short term rehab, and long term care, with a strong focus on providing outstanding customer service and care. Admissions Coordinator Schedule:

  • 8 hour shift
  • Monday to Friday, flex as necessary to meet the needs of the faciity


Admissions Director Responsibilities:

  • Obtains referral information from telephone and walk-in inquiries, including complete clinical and financial information necessary for admission committee approval.
  • Works in conjunction with the business office to ensure financial verification is accurate and complete before the patient is admitted to the facility.
  • Obtains family commitment to tour the facility on 80% of viable referrals. Conducts facility tours with families to ensure their commitment.
  • Conducts the admission process of signing in and explaining admission policies to patients and their families. Ensures a smooth transition is achieved and that all paperwork is complete upon admission.
  • Monitors and updates facility personnel on pending referrals, admissions, and follow-up needed to solidify admission to facility.
  • Informs Marketing Director of any referral source issues that arise during the referral process.
  • Develops relationships with referral sources.


Admissions Coordinator Qualifications:

  • Two to three years experience with the admissions process at a SNF, subacute, rehab, or other health care facility.
  • Excellent organizational skills with a detail orientation towards documentation and follow-up.
  • Professional telephone skills with the ability to handle referral calls effectively and efficiently, meeting the needs of the referral sources and families.
  • Computer experience using a marketing information system.
  • Knowledge of managed care industry preferred
  • HMO initial auth requests


Customer Service:

  • Presents professional image to consumers through dress, behavior and speech.
  • Adheres to Company standards for resolving consumer concerns.
  • Ensures that all patient/resident rights are protected.
