Responsibilities:
* Ensure all teams provide comprehensive and timely intake assessments to all referrals. Guarantee that persons have access to all identified team members during the initial intake process in order to provide a continuum of care (i.e. vocational, medical, psychiatric, recovery, etc.).
* Ensure that teams are coordinated and are overseeing all case management related services for all persons. This includes assisting Persons with managing income, insurance, other entitlements, accessing other services and community resources and scheduling/coordination of all medical and psychiatric appointments.
* Make certain that services are culturally competent, strengths based, recovery based, focused on rehabilitation and are inclusive of Person preferences. Guarantee that the Person is at the center of all care and that all interventions are developed and provided in a manner consistent with Person preferences.
* Ensure that staff members and the team are focused on independence and strength based intervention in order to assist Persons in their goals.
* Ensure that Treatment Plans, assessments and all rehab option related work is focused on person preferences and goals, that the person is at the center of the process and that the interventions are clinically based. Make certain TPs are completed in a timely manner and treatment goals are realistic, attainable, and promote recovery.
* Ensure that crisis management and risk protocols are utilized and that persons. Ensure teams identify risk issues in persons and bring risk issues to the risk management team in a timely manner. Ensure that teams/programs continuously assess risk and implement any risk plan developed by the committee and/or team members in conjunction with the person.
* Provide guidance to teams related to risk.
* Continuously assess training needs of staff and work with supervisors to address identified needs. Assist teams in developing training to address specific team issues, provide training and guidance as appropriate.
* Act as a primary liaison to DMH, other providers, community resources, family members, and others that may be involved in person care.
* Participate in audits of medical files as appropriate.
Act as a backup on call to Team Leader and provide clinical guidance, assistance with management of incidents.
Schedule: Monday-Friday 9am-5pm with on call responsibilities
