Summary Responsible for supporting Strategic Account Managers (SAMs), Social Workers, Volunteers, and Clinical Leadership in maintaining the overall marketing strategic plan within their defined region while supporting the overall organizational programs. Completing outreach and support throughout the defined territory and development of certain strategic relationships as defined by leadership. Works with current referral sources to increase referrals from existing sources, promoting the growth of VIA Health Partners patient census. Responsible for communicating the total value of VIA Health Partners services to new/potential clients/referral sources as well as supporting and managing existing ones. Assisting Team Members with Outreach to Clients/referral sources include (but not limited to) referring physicians/practices, long term care and assisted living communities, and hospital personnel. Works closely with key programs within VIA Health Partners to include community outreach, activities in facility settings, support in communication of patient information to referral sources within the territory. Communicating with clients and referral sources will always maintain positive professional relationships. Makes a significant contribution to the success of VIA Health Partners by educating our existing and new/potential referral sources via various community projects, education programs, process improvements, and special event activities. Supports the overall organizational mission by providing/or coordinating organizational community education.

Essential Functions

  • Support in Management of Existing and New/Potential Clients/Referral Sources
  • Supports SAMs, Social Workers, and Clinical Leaders in Maintaining ongoing support and regular communication to existing as well as new/potential clients and referral sources. Makes contact on a regular and appropriate basis. (Number of contacts (phone/face to face) to be defined: # of face-to-face contacts per week, # of phone contacts per week and # of new referral sources per month).
  • Understands Community Needs by conducting thorough assessments to identify the specific health needs, preferences, and concerns of the community helps tailor outreach efforts to address relevant issues.
  • Works closely with VIA Health Partners team and the client/referral sources in the community in the Strategic Account Managers (SAM’s) determined territory.
  • Works with Leadership and SAMs to plan daily/weekly/monthly activities and goals around regular reports and feedback regarding top, medium and low referral sources as well as client concerns, feedback, or other pertinent information.
  • Assists in organization and preparation for upcoming events and educational opportunities. Organizes marketing collateral in the office setting.

Key Responsibilities

  • Community Outreach
  • Develops and implements outreach strategies to engage local communities, healthcare providers, and referral partners.
  • Organizes and participates in community events, health fairs, and educational presentations to promote hospice and palliative care services.
  • Brand Recognition
  • Collaborates with marketing teams to create promotional materials and campaigns that enhance the organization’s visibility and reputation.
  • Fosters relationships with local media to share success stories and news about our services.
  • Assists with social media marketing opportunities to grow brand recognition and increase social media presence in market.
  • Facility Engagement
  • Conducts regular visits to healthcare facilities and organizations to establish and maintain partnerships.
  • Assists and conducts activities within facility settings to interact with and educate facility residents as to VIA Health Partners while building familiarity with our team and the services we provide.
  • Provides educational services and lectures to healthcare professionals and referral partners about hospice and palliative care services.
  • Meeting Assistance
  • Assists SAMs in scheduling and coordinating meetings with referral partners and community members.
  • Organizes and prepares materials for meetings, presentations, and training sessions.
  • Attends and contributes to partnership meetings to grow the referral base.
  • Identifies and schedules additional outreach within the referral partners we are assigned. Utilizes the monthly budget to meet these needs.
  • Market Strategy Development
  • Analyzes community needs and identifies opportunities for growth and expansion of services.
  • Works with leadership to develop and implement strategies that align with organizational goals.
  • Attends quarterly business development growth meetings with clinical leadership to identify and capitalize on opportunity and gather support for upcoming events.
  • Education and Training
  • Participates in ongoing training and education to stay informed about hospice and palliative care practices and trends.
  • Shares knowledge with team members and community partners to promote best practices in patient care.
  • Attends community-based meetings and events (i.e., Chamber of Commerce, senior resource groups, marketing events held at referral sources, community resource meetings, VIA Health Partners organizational and fundraising events (in SAMs territory to increase referral sources.
  • Stays abreast of all regional communications and provides feedback to leaders on the regional approach of other service providers.
  • Develops clinical knowledge pertaining to Hospice and palliative care to expand VIA Health Partners footprint throughout territory.
  • May perform other duties as required.

Qualifications

Minimum Qualification

  • A bachelor’s degree in a related field is required or equivalent experience in lieu of degree.
  • A minimum of 2 years of healthcare sales experience. Hospice experience a plus.
  • Proficiency in Microsoft Office is required
